The Aftercare portion of the program begins when the youth returns home from the 10 day overnight stay. Adirondack Experience views itself as part of a larger team of agencies and individuals, working together to help youth. AE will make recommendations to local agencies and will assist in connecting participants to other services as needed.
AE Aftercare services include:
1) Providing the youth with a toll free 800# that he or she may use to maintain contact with staff at AE, allowing the staff to provide support and direction with goals that were developed during the 10 day portion of the program. Staff is also available, through the use of the 800# to provide crisis counseling.
2) Providing youth with an opportunity to return to AE six times over the course of one year. Parents, siblings and caseworkers are encouraged to attend and participate in these “ reunion days”. This one-day experience is designed to foster the gains made during the 10 day portion of the program by helping to maintain the relationships that developed between the youth and their peers as well as with specific staff members.
3) Follow-up may also include visits by staff members to the youth’s home or school. Staff use this time to provide support and direction to the youth’s goals as a continuation of the treatment plan goals developed prior to and during the 10 day overnight portion of the program. Staffing of these follow-up visits varies, depending on the type of follow-up being done and the specific activity being offered.
